Background : Cases of severe malnutrition in Indonesia still abundant. Severe malnutrition could be assessed by mid – upper arm circumference (MUAC) and weight for height z-score (WHZ). MUAC as a screening method is expected to find malnutrition at community. Objective : To determine whether MUAC measurements can be used for screening severe malnutrition at community Material : A Cross-sectional study was conducted to 853 children aged 6 – 59 months who came to Integrated Health Post (Posyandu) in Semarang. Anthropometric measurements were performed by cadres and researcher included MUAC, and WHZ. Statistical analysis using Mc Nemar test and no difference when p > 0.05. Sensitivity and spesificity determined by table 2 x 2. The best cut off shown in ROC Results : Eight hundred fifty three out of 1115 children met the criteria, consisting of 419 (49.1%) boys and 434 (50.9%) girls with the most age over 2 years (57.2%). There was no difference measurement between cadres and researcher with Kappa value 0.726. There were differences between measurement of MUAC by cadres with below red line status or WHZ (p < 0.0001, p < 0.0001), MUAC measurements with WHZ (p < 0.0001), and MUAC measurements with WHZ and HAZ (p < 0.0001). Sensitivity, specificity, PPV, and NPV of MUAC measurement by cadres was 12.5%, 99.9%, 75%, 97.5% respectively, while by researcher was 17%, 99.6%, 57%, 97.6% respectively. In this study, MUAC 14 cm was the best cut off for severe malnutrition Conclusions : MUAC measurement couldn’t screen severe malnutrition at community because it was not sensitive
