CHARACTERISTICS ANAPHYLAXIS IN CIPTO MANGUNKUSUMO HOSPITAL

Background : Anaphylaxis is a potentially life-threatening systemic allergic reaction. Multiple etiologies are incriminated in the pathogenesis of anaphylaxis, and these etiologies vary depending on age and geographic zones. Anaphylaxis most commonly affects the cutaneous, respiratory, cardiovascular, and gastrointestinal systems. This study was conducted to identify common causes of anaphylaxis and the characteristics of anaphylactic reaction in Cipto Mangunkusumo Hospital. Material : This was a retrospective case note study of children presenting with anaphylactic reaction over a 5-year period in Cipto Mangunkusumo Hospital, Jakarta. Medical records patients from 2012 to 2017 with a diagnosis of anaphylaxis were manual reviewed. Results : This is the first study of anaphylactic reaction in children reported in Cipto Mangunkusumo Hospital. Twenty episodes of anaphylaxis in 19 patients were included. Most of the patients were male (53%) with median age was 8.6 years. Generalized urticaria (84%) were the most common presenting symptoms and most patients were having first episodes of anaphylactic reactions (95%). The most common trigger was drugs (63%) with 43% were chemotherapy agents and only one patient underwent the desensitization protocol. Other triggers were blood components (32%) and food (5%). All patients were administered intramuscular adrenalin and they all survived. Conclusions : Most anaphylaxis patients in Cipto Mangunkusumo Hospital were having first episodes of anaphylactic reactions. Drugs were the most common causes of anaphylaxis and generalized urticaria is the most common symptoms with intramuscular adrenaline as the first line treatment.
