URINE NEUTROPHIL GELATINASE-ASSOSCIATED LIPOCALINE AS BIOMARKER ACUTE KIDNEY INJURY IN SEPSIS

Background: Sepsis-induced acute kidney injury (AKI) is the most common form of AKI observed in critically ill patients. AKI mortality in septic critically ill patients remains high despite our increasing ability to support vital organ systems. Unfortunately, creatinine is a delayed and unreliable indicator of AKI. Neutrophil gelatinase-associated lipocalin (NGAL) has emerged as a promising non-invasive biomarker of kidney injury. 
Objective: We systematically reviewed the utility urine NGAL measurements for AKI in septic patients.
Methods: This cross sectional study was conducted in Pediatric Intensive Care Unit Wahidin Sudirohusodo hospital from Mei until August 2018. A total 79 sepsis patient were included. Urine specimens were collected at admission.
Results: The initial urine levels of NGAL was significantly increased in AKI groups. Cut off point ?188 ng/ml was obtained through the ROC, with the AUC 0,725, sensitivity 69,2%, specificity 71,2%, positive predictive value 32,1%, and negative predictive value 92,1%.
Conclusion: urinary NGAL level has good validity and could be used test for ruled out AKI on sepsis patients
