FACTORS ASSOCIATED WITH QUALITY OF LIFE IN CHILDREN WITH CONGENITAL HYPOTHYROIDISM

OBJECTIVE: Congenital hypothyroidism is an endocrine disorder commonly observed in children, leads to cognitive and motoric impairment, thus affect the quality of life. The objective is to evaluate several factors that associated with the quality of life in children with congenital hypothyroidism
METHOD: This was a cross sectional study from medical record of 40 congenital hypothyroidism children attending pediatric endocrinology outpatient clinic from March – May 2019, who consumed levothyroxine (L-T4) for at least 3 months. The quality of life were assessed from parents report using Pediatric Quality of Life Inventory (PedsQL) generic scale, which evaluated physical and psychosocial (sum of emotional, social, and school) functioning. Associated factors variables were age at first treatment, length of treatment and initial L-T4 dose. Correlation and logistic regression test were used, with P value <0.05 regarded as significant. 
RESULT: A total of 24/40 children were boys, with median age was 2.95 (2-9)years old. The median age at first treatment, length of treatment, and initial dose of L-T4 were 12(1-80)months, 20(6-54)months, and 50(25-100)mcg. The mean score of PedsQL in physical and psychosocial functioning were 77.68?5.256 and 77.59?3.170, respectively. The age at first treatment had a negative correlation with physical functioning (r -0.497; P<0.001) and psychosocial functioning (r=-0.410; P<0.001). The length of treatment had positive correlation with physical functioning (r=0.9170; P<0.001) and psychosocial functioning (r=0.858; P<0.001). And the initial L-T4 dose had a positive correlation with physical functioning (r=0.540; P<0.001) and psychosocial functioning (r=0.350; P<0.05). The length of treatment had the most significant values that affecting the quality of life (P<0.001).
CONCLUSION: The higher quality of life score in children with congenital hypothyroidism is associated with age at first treatment, initial dose of L-T4, and mostly by the length of treatment.
