Objective:
Myiasis is Dipthera (flies) larval infestation in the living tissue of human or animals within a certain period. Many myiasis case occur in tropical areas especially in lower socio-economic groups. Cutaneous myiasis, together with wound myiasis, is the most frequently encountered clinical form.
Case:
A three-year-old girl, came to Siti Aisyah hospital with a chief complaint of swelling and itching sensation on the both plantar pedis since more than two weeks. The patient had fever and intense itching, swelling, redness with necrotic tissue at the same site. The swelling slowly progressed during the subsequent one week after which her mother noted a small nodule in the middle of inflamed area with one or more maggots within it. Debridement was done, during this procedure, a large number of larvaes were extracted, total of 21 larvaes were removed with forceps. Dressing with hydrogen peroxide 3%, and the wound was consistent dressing change on daily basis with porvidone-iodine and covered with an antibiotic ointment. Analgesic and broad spectrum antibiotics were prescribed.
Conclusion:
Myiasis usually associated with poor personal hygiene and low educational level. Prevention strategies for the patient was avoid being barefoot.The goal of treatment is the complete removal of the larva from the skin, with the prevention and control of secondary infection. To confirm the diagnosis, morphologic identification of Dipthera (flies) should always be carried out by an experienced entomologist. Prognosis of the case was good as the wound healed timely.
