INFANTS BORN TO HIV MOTHERS IN M DJAMIL HOSPITAL, PADANG

OBJECTIVE : Infants and young children who acquired HIV have a high risk of illness and death. HIV can be transmitted from an HIV-positive woman to her child during pregnancy, childbirth and breastfeeding (vertical transmission). In 2017, 180,000 children became HIV positive, the vast majority through vertical transmission. The aim of this study was to determine the profile of infants born to HIV mothers in M Djamil Hospital and follow up in preventing vertical transmission.
METHOD : This was a retrospective study. Data were collected from medical records of Infants born to HIV mothers in M Djamil Hospital from 2011 to 2019. The data were gestational age, mode of delivery, mother’s HIV status, antiretroviral therapy (ART) in mothers, infant’s HIV status, and follow up outcomes. Data is presented descriptively in number and frequency.
RESULT : There were 29 babies born to HIV mothers by SC delivery, fullterm, and got formula milk feeding. Fourteen mothers (49%) were diagnosed with HIV positive based on serological examination during pregnancy and received ART. Only 7 mothers (24%) had CD4 data, which 6 (86%) with CD4 levels > 200 cell/mm3 and 1 (14%) with CD4 < 200 cell/mm3. Of 29 babies, 4 babies were not 6 weeks old yet at the time of observation, 11 babies ( 44%) had undergone virological examination at the age of 6 weeks with the results were 1 positive baby and 10 (90%) negative. Of 10 babies with negative result, 5 babies (50%) underwent second virological examination at 6 months old and the results were still negative. Virological examination was conducted in 6 of 17 patients (35%) before the Jaminan Kesehatan Nasional (JKN) era, and 5 of 8 patients (62%) afterwards. Many patients were lost to follow-up. 
 
CONCLUSION : All mother diagnosed with HIV received ART prophylaxis and underwent SC delivery. There were only 24% of mothers had CD4 data, which 86% of them had CD4> 200 cell/mm3. Besides, 44% of 6 weeks old babies underwent virological examination, and the result is 90% negative, furthermore, only 45% of them continued it at the age of 6 months and also had negative result. The number of patients who underwent virological examination increased after the JKN era (62%). In fact, many patients did not continue to follow-up, and it showed a poor adherence of patients and health provider.
