HEPCIDIN LEVEL FOR DEFINING AND RECOGNIZING DIFFERENT TYPES OF ANEMIA IN CHILDREN WITH CHRONIC KIDNEY DISEASE

Objective : Anemia is common and associated with poor adverse outcome in children with chronic kidney disease (CKD) and it causes an increase of morbidity, mortality and accelerates the rate of progression of CKD. This study aimed to define and recognize the types of anemia by using hepsidin, transferrin saturation and ferritin parameters in children with chronic kidney disease.
Methods : A cross sectional study was conducted in Dr Soetomo Academic Hospital from December 2018 to February 2019. The population of this study was children with pre-dialyis CKD. Laboratory tests including complete blood count, serum hepcidin, transferrin saturation  and serum ferritin were performed.
Results : A Total of 47 children (27 boys vs 20 girls) between 3 months to 18 years old, was diagnosed CKD, not underwent on dialyis and no history of erythropoetin administration and blood transfussion within 3 months, no acute infection and iron supplementation were included. The primary disease that causes of CKD were lupus nephritis (38.3%), nephrotic syndrome (23.4%), urologic disorder (23.4%)  tubulopathy (10.6%) and others (4.3%). There were 26 children CKD anemia and various type of anemia, as follow: mixed anemia 10 (38.5%), renal anemia 6 (23.1%), absolut iron deficiency anemia 6 (23.1%),  and anemia with iron overload 4 (15.3%).
Conclusions : Mixed anemia is the most common type of anemia in children with CKD, followed by renal anemia, absolut iron deficiency anemia and anemia with iron overload. Identification of this type of anemia based on hepcidin, transferrin saturation, and ferritin level in the blood.
