OBJECTIVE: Recurrent rheumatic fever recurrence (RFR) contributes to the worsening of rheumatic valve disease. Cardiac sequel is the main cause of chronic heart disease. Study to describe risk factor for RFR is needed. The objective of this study was to analyze the potential risk factors for RFR in children. METHODS: This retrospective study was collect data from medical record of patient were diagnosed with rheumatic fever under 18 years. Patients were hospitalized in pediatric ward in the child health department of Dr. Soetomo Academic General Hospital from January 2013 to December 2018. RESULT: There were 42 children include this study, the mean age was 10.8 ± 3.46 years, range age 4-17 years old and median age 11 years with the number of events boys and girls respectively 21 children (50%). RFR occurred in 12 (28.8%) children. Prior pulmonary hypertension (PH) present in 6 (60 %) RFR children with OR 6.5 (95% CI 1.3 to 30.4) and p value = 0.012 (p<0.05) and in the frequency of the incidence of RFR is higher in groups with non-compliance to secondary prophylaxis compared with compliance with p values = 0.016 (p<0.05), OR 0.82 (95% CI 0.04 to 0.77). CONCLUSION: Low compliance to secondary prophylaxis and prior PH were risk factor for recurrent attack of rheumatic fever
