CLINICAL MANIFESTATIONS OF INPATIENT CHILDREN WITH HEPATITIS A IN HASAN SADIKIN GENERAL HOSPITAL, BANDUNG, INDONESIA

Objective: Children infected with Hepatitis A only 30% show clinical symptoms while 70% are asymptomatic. Symptomatic form of hepatitis A is usually mild, self-limiting and rarely develops into fulminant hepatitis. Total bilirubin, direct bilirubin, ALT, AST and prothrombin time are predictors for increasing risk developing fulminant hepatic failure. This research aims to describe clinical manifestation of inpatient children with Hepatitis A in Hasan Sadikin General Hospital Bandung
Method: This study is a descriptive retrospective study of patients admitted in Hasan Sadikin General Hospital from January 2014 - December 2018
Results: There were 17 pediatric patients hospitalized due to hepatitis A in Hasan Sadikin General Hospital. There were 12 male (70%) and 5 female (30%), the youngest was 5 years old and the oldest was 15 years old. Symptomps often found were jaundice (94%), fever (94%), nausea (88%), vomiting (65%), anorexia (65%), malaise (65%), dark urine (65%), abdominal pain (53%), diarrhea (35%), light colored stool (23%), itchy (18%), loss of consciousness (12%). The most common causes of hospitalization were vomiting (65%), abdominal pain (23%) and loss of consciousness (12%). Fulminant hepatitis found in 2 childrens. All patients with fulminant hepatitis had total bilirubin of >10.26 mg/dL, direct bilirubin of >6.21 mg/dL, and prothrombin time prolonged more than 27.32 seconds. Half of fulminant hepatitis patients had ALT >1365.7 IU and AST > 1635.78 IU. The shortest treatment period was 3 days and the longest was 25 days. Patients discharged from the hospital with clinical improvement were 88%, and 12 % were dead
Conclusion: Clinical manifestations of inpatients children with Hepatitis A are widely varies. Risk of developing fulminant hepatitis in children is very rare, but if it occurs it has a high mortality rate.
