MANAGEMENT APPROACH OF PYLORIC STRICTURE IN CHILDREN INGESTED CORROSIVE AGENT: A CASE REPORT

OBJECTIVE. Corrosive ingestion is an emergency case that can cause gastrointestinal injury and life threatening within hours after the agents has been ingested. Delayed management could lead into severe complications such as esophageal stricture or pyloric stricture and perforation. This report aims to describe the initial management of corrosive injury and how to prevent further complication. 

CASE. A 14-year old girl hospitalized in Dr. Cipto Mangunkusumo General Hospital because of suffered from persistent vomiting and lost-weight more than 5 kg after accidentally ingested corrosive agents (toilet cleaner) 7 months ago. She then underwent gastrointestinal endoscopic procedure that revealed severe esophagitis with esophageal ulcer, erosive gastritis, gastropathy, laryngopharyngeal reflux, and pyloric stricture. Afterwards, she put on proton pump inhibitors (PPI) and underwent pyloric dilatation. Total parenteral nutrition was initially performed for 3 days besides antibiotics and intravenous steroid for therapy. After the first dilatation, the patient has no more vomiting, no abdominal pain, and she could eat solid food without feeding difficulties. Second dilatation was scheduled two weeks after for evaluation.

CONCLUSION. Proper emergency treatment, PPI, and healthy lifestyle after corrosive ingestion is very important in order to life saving and prevent further complication.
