Abstract Body
Introduction: Haploidentical hematopoietic stem cell transplantation (haplo-HSCT) is an important curative strategy for pediatric acute myeloid leukemia (AML), but evidence on the effect of donor age is limited. Objectives: To evaluate the impact of related haploidentical donor age on engraftment, GVHD, relapse, and survival in children with AML. Methods: We retrospectively analyzed pediatric AML patients receiving related haplo-HSCT at our center, grouped by donor age ≤35 vs >35 years. Outcomes were compared and prognostic factors assessed using multivariable Cox regression. Results: In total, 241 patients were included (≤35: n=165; >35: n=76). Recipients in the ≤35 group were younger (median 75 months [9.47–185] vs 130 months [20–205]); other baseline variables and transplant strategies (sex, ABO, remission/MRD, conditioning, GVHD prophylaxis) were comparable. Median follow-up was 24 months (1–90). Engraftment was similar between groups. The ≤35 donor group had better OS (87 .54%, 95%CI 0.824–0.930 vs 71.84%, 0.607–0.850; P=0.018), RFS (83.9%, 0.782–0.899 vs 70.5%, 0.596–0.833; P=0.047), and GRFS (79.1%, 0.729–0.857 vs 61.6%, 0.505–0.752; P=0.010). Grade II–IV aGVHD (27 .9% vs 28.3%, P=0.948), CIR (11.9% vs 17 .4%, P=0.357), and NRM (8.7% vs 16.7%, P=0.077) did not differ. On multivariable analysis, donor age >35 independently predicted worse OS (HR 1.041, 95%CI 1.004–1.079; P=0.030) and GRFS (HR 1.038, 95%CI 1.006–1.064; P=0.023). Conclusions: Donor age ≤35 years was associated with superior OS, RFS, and GRFS without higher aGVHD or delayed engraftment, supporting preference for younger related haploidentical donors when options exist.
