Accuracy of Urinalysis for Urinary Tract Infection in 6 months to 18 Years old Children along with its Bacteriological Profile

Background : Urinary tract infection (UTI) is one of the most common pediatric infection. Gold standard is urine culture which results will be seen by days though prompt treatment is needed. Material : Urine culture and urinalysis results of children age 6 months to 18 years during Mei 2017 to April 2018 were obtained from database. Urine culture results was then paired with urinalysis result taken not more than 2 days differences. Results : Eighty paired data were obtained with 33 additional urine culture data without paired urinalysis. Median age of paired data was 36 month (6 to 192 months old), 45 of them were boys. Accuracy of leucocyte esterase(LE), nitrite, LE or Nitrite, microscopy (white blood cell), microscopy (bacteria), and LE or nitrite or microscopy was respectively 70%(59.96 to 80.04), 82,5%(74.17 to 90.83), 73.75%(64.11 to 83.39), 61.25%(50.57 to 71.93), 67.5%(57.24 to 77.76), 56.25(45.38 to 67.12). LE or nitrite or microscopy has highest sensitivity (47%;95%CI 26.17 to 69.04) while nitrite has highest specificity (100%;95% CI 94.25 to 100). Eighty-four out of 112 culture data were sterile. Eschereria coli (14/28) was most frequently found followed with Pseudomonas aeruginosa (5/28). Highest susceptible antibiotics were amikacin (25/28), meropenem (24/28), and Gentamycin (18/28) while susceptibility of Cotrimoxazole, Ciprofloxacin, and Amoxicillin were 10/28, 18/28, and 3/28. Conclusions : Nitrite test has highest accuracy and specificity though combination of LE or Nitrite test or microscopy test need to be accounted to increase sensitivity. Amikacin has the highest susceptibility for intravenous antibiotic and Ciprofloxacin for oral antibiotic.
