Background : Sepsis caused more than 4,300 deaths in year and contributed to approximately 7% of total mortality rate in children. Several studies on critically ill patients showed that microalbuminuria could possibly be a beneficial marker to predict outcome. Objective to find out the relationship of urinary albumin to creatinine ratio and mortality in children with sepsis. Material : An observational analytical study with a prospective cohort approach. The population were the children admitted to PICU Prof.Dr.R.D.Kandou Hospital from October - December 2017 diagnosed with sepsis. The inclusion criteria, age of 1 month to 72 month fulfilled the sepsis criteria, and the parents agreed to signed the inform consent. The obesity, malnutrition, previous history of glomerulopathy, hypertension, malignancy, diabetes mellitus, chronic kidney disease, trauma and post-operative, and failure to collect urine sample in 6 hours were excluded. The data were collected and analyzed using SPSS version 23. Results : From 41 children with sepsis, the youngest age was 1 month with the oldest age was 65 months, consisted of 24 boys. Total sample who survived 21 children, and not survived 20 children, consist of 11 boys and 9 girls. The survived group had the mean age of 21.4 months, whereas the group who did not survive had the mean age of 19.6 months. The mean of urinary albumin to creatinine ratio in children with sepsis was 2,384.2 mg/g. Children with sepsis who survived had the mean urinary albumin to creatinine ratio 343.9 mg/g, but in the other group, the mean urinary albumin to creatinine ratio 4,526.5 mg/g. There was a significant relationship between urinary albumin to creatinine ratio and sepsis mortality in children (p=0.004). Conclusions : The higher level of urinary albumin to creatinine ratio, the higher mortality rate in children with sepsis.
