The Prevalence of electrocardiogram abnormalities in obese adolescents

Background : Obesity in adolescents is a known risk factor for cardiovascular disease mortality and sudden cardiac death. Obesity is associated with a wide variety of electrocardiographic (ECG) abnormalities. Material : a cross sectional study conducted at Children Cardiology Clinic – Integrated Heart Center of Sanglah General Hospital Denpasar, by recording ECG of obese adolescents aged 11-15 year old in several junior high school from Desember 2016 until April 2017. This study was approved by the Research Ethics Committee at Udayana University Medical School, Sanglah Hospital, Denpasar. Results : a total of 78 ECGs of obese adolescents (60% male) were selected. The mean weight and age was 82.6 (SD 15.2) kg and 13.2 (SD 1) years respectively. Pre-hypertension was found to be present in 25 subjects (32%) while 18 subjects (23%) were in hypertension state. There were 29 subjects (37.2%) found to have abnormal ECGs. Sinus tachycardia was present in 13 subjects (16.7%), and sinus arrhythmia was identified in 11 subjects (14.1%). Eight patients (10.3%) experienced prolongation of QTc interval and 5 patients (6.4%) presented with prolongation of PR interval. There was no shifts of the P wave, QRS wave and T wave axes, changes of P wave morphology, low QRS voltage, T wave flattening, ventricular enlargement and ST segment changes found in this study. Conclusions : the prevalence of cardiac abnormalities based on ECG examination in obese adolescents was 37.2%, consist of abnormality of heart rhythm, prolonged PR interval and prolonged QTc interval.
