Echocardiographic Profile of Pediatric Systemic Lupus Erythematosus at Cipto Mangunkusumo Hospital, Jakarta

Background : Background. Cardiac involvement is one of the main complications substantially contributing to the morbidity and mortality of patients suffering from systemic autoimmune diseases. Systemic lupus erythematosus often involves the heart, mostly the pericardium usually manifests as diffuse pericardial effusion (PE). The aim of this study is to determine the echocardiography profile of children with systemic lupus erythematosus (SLE) on the first echocardiogram at Cipto Mangunkusumo Hospital, Jakarta Material : Methods: Patients who fulfilled the 1997 revised American College of Rheumatology criteria for SLE were included during January 2014- April 2018. All patients underwent echocardiographic study. Results : Results: One hundred and six patients with SLE were evaluated over a period of 4 years and 3 months consisted of 13 boys and 93 girls, age 8-18 years. Normal echocardiographic profile were seen in 62% patients (66) while 38% (50) showed abnormal echocardiogram consisted of pericardial effusion, left ventricular dysfunction, mitral regurgitation, and pulmonary hypertension. Pericardial effusion was the commonest initial manifestation, seen in 30% (32) patients and one of them had massive pericardial effusion. Left ventricular dysfunction was seen in 7.5 % patients (8), mitral regurgitation in 14 % patients (15) while mild and moderate pulmonary hypertension in 13 % (14) and 2.8 % (3) respectively. Median of ejection fraction and fractional shortening were 64% and 34% respectively. Clinical cardiac manifestation showed dyspnea only in 1 child with massive pleural effusion. Conclusions : Conclusion: SLE is a multisystem disorder affecting predominantly young females. Pericardial effusion was the most common cardiac manifestation. But most of them have no symptoms.
