Short term outcome of invasive and non invasive neonatal mechanical ventilation

Background : Mechanical ventilation has become a must to enhance neonatal survival. Both invasive and noninvasive mechanical ventilation techniques are inherent to the care of most patients admitted to neonatal intensive care units. This study was undertaken to analyze the common indication and outcome of neonates required both invasive and non invasive mechanical ventilation.
Material : This was a retrospective observational study conducted on neonates admitted in Neonatal Intensive Care Unit (NICU) that required invasive, non invasive or both of mechanical ventilation over a period of 3 months (January 2018 to March 2018).
Results : A total of 13 neonates were mechanically ventilated during the study of period of which 69.23% (n=9) was survived. The majority of the subjects (84.6%) were referral patients. The survival rate was higher in aterm neonates (88.89%). The neonates with birth weight < 2500 had higher mortality rate (75%). Respiratory Distress Syndrome was the most common indication for preterm neonates (75%), while neonatal pneumonia was the most common indication for aterm neonates (55%), followed by sepsis (33%) and meconeal aspiration syndrome (11%). Weight <2500 g, gestation <34 weeks, initial pH <7.1, initial pCO2 >60 mmHg, presence of shock were significantly associated with mortality (p<0.005). Sepsis, asphyxia, initial pO2, age of referral did not have a significant association with mortality. There were no significant difference of length of stay between neonatal using invasive, non invasive or both of mechanical ventilation.
Conclusions : Weight <2500 g, gestation <34 weeks, initial pH<7.1, initial pCO2 >60 mmHg, presence of shock were significantly associated with mortality in ventilated neonates.
