THE HISTOLOGICAL PATTERN OF PRIMARY GLOMERULAR DISEASE AMONG CHILDREN AT DR KARIADI HOSPITAL

Background : A percutaneus renal biopsy by ultrasound guidance is a procedure to obtain a small part of renal tissue. The clinico-histopathological especially glomerular or interstitial pathologies is correlation with prognostic and maybe to consider the treatment by clinician. The aim of this study was to describe the histological pattern of kidney biopsy in children with nephrotic syndrome (NS) in Dr. Kariadi Hospital, Semarang.
Material : Retrospective study were conducted from medical record of NS patient who underwent kidney biopsy from March 2011 to May 2018. The kidney biopsy was performed by ultrasound guidance and biopsy needle 18G. The data was collected based on number of glomerular and pattern of kidney biopsy by light microscopy and analyze by descriptive.
Results : There were 42 patients who indicated kidney biopsy consist of steroid resistant (37), frequent relapse NS (2) congenital NS (2) and dependent steroid NS (1). Rasio male to female was 1:1. The mean age was 9.678 years (SD 4.7), with minimum and maximum of age were 1.95 and 17.58 years. The mean number of glomerulus was 14.2 (SD 7.57). The histological patterns showed 11 patients with focal segmental glomerulosclerosis (FSGS), 5 patients with minimal change disease (MCD), 3 patients with membranous glomerulonephritis (MGN), membranoproliferative glomerulonephritis (MPGN), focal mesangeal hiperselularity in 2 patient and 16 of 42 patients had interstitial nephritis. 19 patients had similar pattern need immunofluorescent examination to differentiate IgA nephropathy, class II lupus nephritis, nephritis interstitialis and resolving post infection of glomerular.
Conclusions : The most common histological pattern of kidney biopsy in children with nephrotic syndrome was FSGS and MCD respectively.
