CORRELATION OF CORTISOL SERUM WITH SCORE FOR NEONATAL ACUTE PHYSIOLOGY PERINATAL EXTENSION II (SNAPPE II) IN NEONATUS TREATED AT NICU

Background : The severity of the disease is now associated with serum cortisol levels as hormonal response to stress. The purpose of this study was to prove the correlation of serum neonate cortisol levels with SNAPPE II values as a description of the severity of the disease when admission at NICU.
Material : This study used cross-sectional analytic study design and correlation test. This study was conducted at NICU Sanglah Hospital Denpasar, the period September 2016 to February 2017. Samples that met the inclusion and exclusion criteria were assessed by SNAPPE II and had collected the blood sample to measure serum cortisol levels. The Spearman correlation test was used to determine the strength of the correlation between the two variable because one of the data distribution was not normal.
Results : Forty subjects underwent examination of serum cortisol levels and the severity of the disease was assessed when admitted to NICU by SNAPPE II. The median of critically ill neonatal serum cortisol levels in this study was 14.97 (IQR 3.15-26.79) ng/mL. The mean of SNAPPE II in this study was 19.08 (SD 9.16). In Spearman correlation analysis, there was a significant positive correlation between serum cortisol level with SNAPPE II (r = 0.45; p = 0.003).
Conclusions : The conclusion of this study showed a moderate positive correlation between serum cortisol levels and the severity of the disease in critically ill neonates in NICU. Further research is needed to determine the association between serum cortisol levels with the severity of the disease in critically ill neonates by comparing uncritically critical children as controls.
