EVALUATION OF TRANSCUTANEOUS BILIRUBINOMETER ACCURACY IN NEONATAL JAUNDICE

Background : Transcutaneous bilirubinometer (TCB) is a noninvasive, quick and painless method for measuring bilirubin level in neonates. We evaluated the accuracy of this simple measurements for checking serum bilirubin level in neonatal jaundice.
Material : Transcutaneous and serum bilirubin measurements were compared on neonates with jaundice recruited during 01 November 2017 to 30 April 2018 at Neonatal Unit Dr Kariadi Hospital. TCB was measured using a bilirubinometer three times on the forehead and mean levels were calculated. After 30 minutes blood samples were obtained to determine serum bilirubin level. Specifity, sensitivity, positive predictive value and negative predictive value of the transcutaneous bilirubinometer were calculated including ROC curves to asses the accuracy of the diagnostic test.
Results : Of the 142 neonates, 47.9% (n=68) were male and 52.1% (n=74) were female. Median gestational age was 35 weeks, 51% (n=73) were preterm. The range of serum bilirubin level was 5.63-31.05 mg/dL, transcutaneous bilirubin level ranged between 5.40-28.20. Transcutaneous bilirubinometer had a sensitivity, specifity, positive predictive value and negative predictive value of 93 %, 76 %, 79 %, and 92 % respectively. The Bland-Altman error plot demonstrated confirmation between the two methods. The ROC curved showed excellent accuracy of the diagnostic test.
Conclusions : Bilirubin level obtained by transcutaneous bilirubinometer were sensitive bu not spesific compared with laboratory level thus proving the fact that transcutaneous bilirubinometer is an useful device to measure bilirubin in neonatal jaundice.
