THE EFFECT OF BREAST MILK ODOR IN STABILIZING BLOOD GLUCOSE AND SHORTENING TRANSITION TIME TO FULL ORAL FEEDING ON PRETERM NEONATES

Background : Preterm neonates are particularly highly prone to instable metabolic condition. Different studies have shown the use of olfactory stimuli in reducing pain, stabilizing blood glucose and vital sign, and stimulate transition from gavage to oral feeding. The olfactory sense is active since embryonic period. After birth, it affects digestive behaviors, such as sucking, through sensory signal received by the brain stem and thalamus. The main purpose of this study was to investigate the effect of breast milk odor stimulation in stabilizing blood glucose level and in shortening transition time to achieved full oral feeding.
Material : Thirty preterm infants born between 28 weeks and 35 weeks + 6 day gestation and being more than 5 days postnatal age were classified into two groups: breast milk odor group and formula milk odor group. Each group was exposed to the odor everyday. We observe the random blood glucose and time to achieved full oral feeding in each group.
Results : Our study included 17 neonates in the breast milk odor group and 13 in the formula milk odor group. Statistical analysis showed that mean random blood glucose in breast milk odor group significantly higher than in the formula milk odor group (83.94 mg/dl compared to 78.77 mg/dl, p 0.001). Breast milk odor group also has a shorter transition time from gavage to full oral feeding compared to formula milk odor group (3.71 days compared to 4.23 days).
Conclusions : Breast milk odor stimulation can stabilize blood glucose level on preterm and shorten the transition time to achieve full oral feeding.
