PERCUTANEUS RENAL BIOPSY, TISSUE ADEQUACY AND COMPLICATIONS BASED ON BIOPSY NEEDLE

Background : Percutaneous renal biopsy by ultrasound guiding is the most common method for obtaining tissues for the kidney. There are confusing about safety and adequacy of the sample. The objective: to analyzed safety and tissue adequacy based on biopsy needle 16G and 18G.
Material : A retrospective study was conducted from March 2011 to May 2018. Descriptive analytic was done to analyze the amount of glomerulus, time of puncture and complication. Each group A(16G) and B(18G) was divided by 3 class: class I was glomerulus less than 10, class II was glomerulus 10 – less than 20, class III was 20 or more. Adequacy glomerulus is achieved if > 20 glomerulus.
Results : A total 64 subject consist of male and female 1:1.9; forty six of 64 patients used needle biopsy18G. Mean of ages was 10.74 years (SD4.34).The most diagnosis pre biopsy were steroid resistant nephrotic syndrome (SRNS,37), nephritis lupus (21), frequency relapse NS(2), congenital NS (2), dependent steroid NS (1) and nephritis Henoch Schonlein Purpura (1). In group A, mean of glomerulus 14.1(SD7.57) and the presentation of glomerulus in class II 55.6%, class III and class I 22.2% compared to group B mean of glomerulus 16.8(SD7.69), and class II 52.2%, class III 34% and class I 13%. The mean of puncture was 2.3(SD0.55) similar in both group. One patient of group A had a serious complication cause of blood clot in bladder, but after cystostomy by surgeon was recovered. Macroscopic hematuria was found in 2 patients on each group, microscopic hematuria revealed 3 in group A and 2 in group B. By Mann-Whitney U test there were not significant different between the needle of biopsy and amount of glomerulus (p=0.203) and times of puncture (p=0.776).
Conclusions : Needle 18G was safer than 16G. Adequacy of tissue was achieved 34 % by needle 18G.
