RELATIONSHIP BETWEEN NUMBER OF RISK FACTORS TO CARDIOVASCULAR ABNORMALITY IN PEDIATRIC CHRONIC KIDNEY DISEASE AT M DJAMIL PADANG HOSPITAL

Background : Cardiovascular diseases (CVD) are the common cause of morbidity and mortality in peadiatric chronic kidney disease (CKD). Left ventricular hypertrophy (LVH) and diastolic dysfunction are early markers. The aims of this study were to evaluate (1) relationship between CKD staging with LVH and cardiac dysfunction, (2) determine how many risk factor are needed to cause CVD in patients with CKD. Material : A cross sectional study of 27 subject with CKD of various stages in pediatric ward and outpatient at M Djamil Hospital Padang from January 2016 – Mei 2018. Physical examination, urinalysis, routine hematology, ureum and creatinin (eGFR), lipid profile and echocardiography were noted. Statistical analysis used are frequency distribution, mean, Fisher’s exact, Kruskal Wallis, Kolmogorov Smirnov and ANOVA tests. p <0,05 is stastisfically significant. Results : Mean (SD) of age was 9,55 (4,10) years. Boys : girls = 1,4:1. Subject with stage 1-5 CKD were 7, 4, 5, 1 and 10 subjects, respectively. LVH was found in 5 of 7 subject with CKD stage 1. Totally about 19 subjects (70,4%) with LVH, most of them in CKD stage 5, there was no relationship between CKD staging and LVH (p=0,419). Diastolic dysfunction was found on 11 of 21 subjects include stage 1, no relationship to CKD staging (p=0,419). Factors affecting the progression of CKD are hypertension (p=0,049) and anemia (p=0,004). There was a significant relationship between the number of CVD risk factors with LVH on CKD patients (p=0,001). Conclusions : In stage 1 CKD has been found LVH and diastolic dysfunction. Most of subject were LVH, but no relationship between CKD staging and LVH. Diastolic dysfunction is earlier found and most common as cardiac dysfunction, but no relationship between CKD staging and cardiac dysfunction. There was a significant relationship between the number of CVD risk factors with LVH in CKD patients.
