RISK FACTOR OF ACUTE KIDNEY INJURY IN CHILDREN WITH IDIOPATHIC NEPHROTIC SYNDOME

Background : Acute kidney injury (AKI) is a clinical complication in idiopathic nephrotic syndrome (INS) that is constituted by a rapid decline in renal function. However, the risk factor of AKI is still uncertain. The aim of this study is to determine risk factor of AKI in children with INS. Material : A retrospective study of children with INS attending pediatric outpatient clinic of Dr. Soetomo Hospital, Surabaya was conducted from March 2011 to December 2017. Children ? 18 years were included and children with secondary nephrotic syndrome was excluded. Estimated glomerular filtration rate was calculated using the Schwartz formula. The diagnosis of AKI was established by pediatric RIFLE. Variable of anemia, obesity, hypertension, cyclosporine A, infection, and antibiotic exposure were analyzed by logistic regression to determine the significant risk factor for AKI in INS with p<0.05. Results : A total of 101 children with INS, 28 (27.7%) children had AKI, in which AKI stage risk in 13 (12,9%), AKI stage injury in 11 (10,9%), and AKI stage failure in 4 (4%). The mean age was 7,2 (SD 4) years and male were 79 (78.2%). Eight (38.1%) children had steroid-resistant nephrotic syndrome and 5 (31,2%) had steroid-dependent nephrotic syndrome. Anemia was the risk factor of AKI in children with INS (odds ratio 10.098; 95% CI 2.429 – 41.985; P=0.001). Conclusions : Anemia is the risk factor for AKI in children with INS.
