BLOOD LEVELS OF 25-HYDROXYVITAMIN D IN CHILDREN WITH IDIOPATHIC NEPHROTIC SYNDROME

Background : Idiopathic nephrotic syndrome (INS) in children is a significant risk factor for vitamin D deficiency because of urinary vitamin D binding protein loss. Information on vitamin D status in INS children is limited. This study aimed to compare blood levels of 25-hydroxyvitamin D (25-OHD) between healthy children, remission, relapse, and steroid-resistant INS.
Material : A cross-sectional study has been conducted of 1-18 years old children with INS in Dr. Soetomo Hospital from January to May 2018. Healthy children, remission, relapse, and steroid-resistant INS group were compared. Peripheral blood 25-OHD levels were measured using enzyme-linked fluorescence assay method with VIDASĀ® reagent. Vitamin D status was defined 25-OHD levels according to International Osteoporosis Foundation (deficiency <20 ng/ml, insufficiency 20-29 ng/ml, and sufficiency >29 ng/ml). The comparison between groups was tested using ANOVA analysis (P<0.05 was considered significant).
Results : A total of 40 children, 23(58%) were boys, participated in the study with the mean age was 10.6 (SD 1.59) years. Vitamin D deficiency was identified in 2(20%) healthy children, 3(30%) remission, 5(50%) relapse, and 7(70%) steroid resistant INS group. The mean of 25-OHD levels in healthy children, remission, relapse, and steroid-resistant INS group were 28.6 (SD 6.22) ng/ml, 24.1 (SD 4.63) ng/ml, 22.1 (SD 4,72) ng/ml, and 19.2 (SD 2,53) ng/ml. There was a significant difference between groups (95% confidence interval, P = 0.01).
Conclusions : Children with INS have low levels of 25-OHD, however, steroid-resistant INS is the lowest levels of 25-OH than remission and relapse INS. The study of vitamin D supplementation in INS children are needed.
