CLINICAL SYMPTOMS AND INTERPRETATION OF LCS ANALYSIS IN PATIENTS WITH DECREASED CONSCIOUS LEVEL IN ABDUL WAHAB SJAHRANIE HOSPITAL

Background : Reduced conscious level in children is a presenting problem which often causes concern for parents and health professionals. The differential diagnosis of it includes all known and unknown diseases in their severest form. The symptoms and signs of the more common causes often overlap, making clinical judgments difficult, which can make the difficulty in giving therapy. Material : Retrospective descriptive study in children with complaint of decreased conscious level before being given antibiotics in Pediatric Department, between January 2016 to December 2017. Data was collected with regards to gender, age, fever, meningeal sign, type and duration of seizure, past history of seizure, old cough, and LCS analysis. Inclusion criteria was children with decreased conscious level suspected of infection. Exclusion criteria was patient with decreased conscious level caused by head trauma and status epilepticus caused by epilepsy. Results : From 41 descriptive study, the majority occurred in male 58,5% and female, 41,5% in the age <2 years 31,7%, range 2 to 5 years 48,8%, >5 years 19,5%. There were 68,3% had fever, 64,29% no meningeal sign at the age group >2 years old. According to type of seizure, we found 78,1% general seizure and 21,9% focal seizure with 65,86% duration of seizures >15 minutes and 34,14% <15 minutes. We found 63,42% with past history of seizure and 36,58% didn’t have it, 82,93% no old cough and 17,07% had it. The LCS analysis are 60,97% viral infection, 24,39% bacterial, 14,63% normal. Conclusions : The clinical symptoms of patients with decreased conscious level are male, age range 2 to 5 years, had fever, no meningeal sign, general seizure type, >15 minutes of seizure duration, had past history of seizure, no old cough, delays and the majority of viral infection in LCS analysis. By knowing early diagnosis, we can provide the right treatment.
