BLOOD LEVELS OF 25-HYDROXYVITAMIN D IN EPILEPTIC CHILDREN WITH SINGLE AND MULTIPLE ANTIEPILEPTIC DRUGS THERAPY

Background : Long-term use of antiepileptic drugs (AED) is a significant risk factor for vitamin D deficiency in children with epilepsy. Differentiation effect between single and multiple AED is still unclear. The aim of our study is to compare the influence of AED treatment for 25-hydroxyvitamin D (25-OHD) levels between single and multiple AED. Material : A cross-sectional study of 2-18 years old epileptic children with long-term AED (phenytoin, phenobarbital, and valproic acid) for >6 months in Dr. Soetomo Hospital from January to May 2018. Single and multiple AED group were compared. Peripheral blood 25-OHD levels were measured using enzyme-linked fluorescence assay method with VIDASĀ® reagent. Vitamin D status was defined 25-OHD level according to International Osteoporosis Foundation (deficiency <20 ng/ml, insufficiency 20-29 ng/ml, and sufficiency >29 ng/ml). The comparison between groups was tested using t-test. (P<0.05 was considered significant). Results : A total of 30 children, 17/30 were boys, participated in the study with the mean age was 7.3 (SD 3.33) years. Vitamin D deficiency was identified in 2/15 single AED and 8/15 in multiple AED group. The mean of 25-OHD levels in single and multiple AED group were 26.6 (SD 5.29) ng/ml and 20.2 (SD 4,25) ng/ml. There was a significant difference between groups (mean difference 6.1 ng/ml, 95% confidence interval, P=0.001). Conclusions : Children with single and multiple AED have low levels of 25-OHD, however, multiple AED is lower than single AED. The study of vitamin D supplementation in epileptic children with long-term AED is needed.
