PREVALENCE AND RISK FACTOR OF HOSPITAL MALNUTRITION IN PEDIATRIC WARD DR SOETOMO HOSPITAL AFTER PEDIATRIC NUTRITION CARE MANAGEMENT

Background : Pediatric Nutrition Care (PNC) is a health service to prevent hospital malnutrition. Prevalence of hospital malnutrition in Dr. Soetomo Hospital before PNC applied (before 2011) was 65%. There is no data on prevalence of hospital malnutrition after PNC is applied. The aim of our study is to determine the prevalence and risk factor of hospital malnutrition in children after PNC management. Material : An observasional retrospective study at pediatric ward Dr.Soetomo Hospital was conducted from May 2017 to May 2018. Anthropometric measurements were obtained twice, at admission and discharge using WHO 2009 for 1 month - 5 years old or CDC 2000 for ? 5 years old. Hospital malnutrition was defined as weight loss ? 2% for length of stay (LOS) <7 days, ?5% for 7-30 days and ? 10% for 1-6 months. Data was analyzed using chi-square test with p<0.05 considered significant. Results : There were 1160 children enrolled in this study. Hospital malnutrition was occurred in 76 (6.6%) children. Most of it were in the weight loss ? 2% for LOS <7 days as many as 40 (52.5%) children while 9 (12%) children were in the weight loss ? 10% for 1-6 months. Fifty seven (60.5%) children were male, fifty seven (75%) children were age 1 month – 5 years and fourty two (55.26%) children have good nutritional. There were significant risk factors in 1 month – 5 years age (8.3%) (OR=2.15 95%CI 1.259-3.656, p = 0.002), infection disease (OR=2.02 95%CI 1.255-3.247, p = 0.002), gastroenterology disease (OR=11.15 95%CI 3.748-33.181, p< 0.001), respiratory disease (OR=9.50 95%CI 3.253-27.772, p <0.001) and hematooncology disease (OR= 6.26 95%CI 2.174-18.033, p< 0.001). Conclusions : Children age range 1-60 months with infection, gastroenterology, respiratory or hematooncology disease have more risk factors of hospital malnutrition after PNC management.
