COMPARISON OF NUTRITIONAL STATUS AMONG CHILDREN WITH BILIARY ATRESIA ACCORDING TO AGE WHEN UNDERGO KASAI SURGERY

Background : Recent studies revealed better outcomes among children with biliary atresia (BA) who underwent Kasai surgery in 60 to 90 days of age. Time delay of Kasai surgery impacts on high risk of complication, including nutritional deficit which leads to malnutrition. The aim of this study is to determine nutritional status of children with BA according to age when the chidren undergo Kasai surgery.
Material : We conducted a cross sectional study among BA children based on magnetic resonance cholangiopancreatography (MRCP) or intraoperative cholangiography who admitted between January 2015- December 2017 and underwent Kasai surgery at Dr. Sardjito General Hospital, Yogyakarta using medical record. The nutritional status was evaluated on day before surgery, and we classified into 4 groups based on age when children undergo Kasai surgery (<60 days, 60-90 days, 90-120 days, and >120 days).
Results : A total of 39 BA children underwent Kasai surgery. Of 3 children who underwent Kasai surgery during <60 days of life had 2 (67%) wellnourished and 1 (33%) malnourished. Twelve children who underwent Kasai surgery during 60-90 days of life had 6 (50%) wellnourished and 6 (50%) malnourished. Seven children who underwent Kasai surgery 90-120 days of life had 4 (57%) wellnourished and 3 (43%) malnourished. Seventeen children who underwent Kasai surgery >120 days of life had 5 (29%) wellnourished and 12 (71%) malnourished.
Conclusions : We report a highest prevalence of malnutrition in biliary atresia children who undergo Kasai surgery >120 days of life. Kasai surgery are recommended to undergo within 60-90 days of life to attain better outcome.
