CORRELATION BETWEEN ZINC PLASMA LEVEL TO INFLAMMATION RESPONSE IN PEDIATRIC INTENSIVE CARE UNIT, DOES BREASTFEEDING PLAY A ROLE

Background : Partial breastfeeding during early life and the use of formula milk with low zinc bioavailability, can lead into zinc deficiency state. Infants with zinc deficiency in general will have higher morbidity, but only a few study evaluated this condition, mainly in Pediatric Intensive Care Unit (PICU). The main objective of this study was to comprehend the correlation of plasma zinc levels with inflammatory responses of 1 month - 2-year-old children admitted in PICU and looking forward about exclusive breast feeding rule in PICU Material : This was a prospective study in a period of January – December 2017 in the PICU Sanglah Central Hospital. Children aged 28 days - 2 years and have an estimated length of stay ? 72 hours, are included. Level of zinc and proinflammatory cytokines of the children were examined in the first 24 hours and after 72 hours. Pearson analysis was used to determine the correlation of variables plasma zinc with levels of proinflammatory cytokines (IL-6 and TNF-?) as a primary outcome and chi-square test for secondary outcome Results : More than 70% sample is insufficiency zinc for the first 24 hours and after 72 hours. Correlation between plasma zinc and proinflammatory cytokines TNF-? and IL-6 is r: 0.006; p=0.97; r: -0.524; p <0.01, respectively in 24 hours and TNF-? and IL-6 is r: -0.99 p=0.6; r: -0.16 p=0.42, respectively in 72 hours. There was a significant association between zinc plasma level in 24 hours with timing to enteral feeding with p?0.05. Exclusive breast feeding have significant association with zinc plasma level in 24 hours with p=0.04 Conclusions : There was moderate negative correlation between zinc plasma insufficiency with proinflammatory cytokines IL-6 in the first 24 hours. Zinc plasma level in 24 hours have association with time to enteral feeding and exclusive breast feeding, but no significant association after 72 hours
