ANION GAP OF PEDIATRIC CRITICALLY ILL IN PEDIATRIC INTENSIVE CARE UNIT

Background : Anion gap (AG) is the most common used biomarkers and the simplest tool to evaluate the acid-base status of patient. High AG in adult patient is associated with increased mortality compared to normal AG. There is a lack data of pediatric patient concerning relationship between AG and mortality. The aim of this study was to investigate the anion gap of pediatric critically ill in Pediatric Intensive Care Unit (PICU). Material : A retrospective study was conducted in PICU Dr. Soetomo Hospital between January until December 2017. All patients admitted to PICU were included. Demographic data and laboratory examination were collected from medical chart. Anion gap was calculated with formula AG = (Na+) - (Cl- + HCO3-) with normal range 8-16 meq/L. Statistical analysis was done using the Chi-square test with significantly P<0.05. Results : A total of 363 children admitted in PICU and only 277 children were included. There were 165 (59.57% ) males and the mean age was 57.2 (SD 3.61) months. The mean AG was 17.71 (SD 0.393) meq/L. As many as 90 (32.49%) patients died in PICU. Children with AG more than 16 meq/L has significant higher mortality than normal AG (OR 1.74 95%CI 1.04 to 2.90, P=0.034). Conclusions : Pediatric critically ill with higher anion gap have higher mortality than normal anion gap.
