TESTICULAR TUBERCULOSIS CASE REPORT

Background : Tuberculosis (TB) is a systemic infection caused by the acid-fast bacterium Mycobacterium tuberculosis. TB is transmitted by coughed aerosol and presents with respiratory symptoms. In addition to infecting the lungs, mycobacterium tuberculosis can also infect other organ systems through the spread of the circulatory system and lymphatics. The incidence of extrapulmonary TB is approximately 20–25%, of which testicular TB is only 3%.
Case Presentation Summary : A boy 7 years old admitted to Community Center for Lung Health Makassar with painless swelling of the right scrotum since 5 years before admitted, increased in size and became ulcerated foci. History of persistent cough, prolonged fever and contact with adult tuberculous patient. On physical examination, the patient was conscious, undernourished, lymphadenopathy on submandibula region dextra et sinistra. Enlarged right testicle, smooth on palpation, soft consistency, swelling, painless, transilluminable. Laboratory result showed positive tuberculin skin test. Chest X-Ray revealed thorax and cardiovascular within normal limits. Ultrasonography revealed hidrocele of the right testicle. Histopathology revealed granuloma focused in central necrotic areas, with conclution chronic inflammation specific et causa tuberculosis. This patient treated with antituberculosis drugs for 6 months.
Learning Points/Discussion : -Testicular tuberculosis is a very rare disease which present as a painless testicular swelling and may mimic testicular neoplasm. -Anti-tuberculous treatment gives a good response. -Early diagnosis will prevent further complications
