MANAGEMENT AND OUTCOME OF PATIENTS WITH INITIAL ASSESSMENT OF BRONCHIOLITIS IN DR KARIADI HOSPITAL

Background : Bronchiolitis is a common acute viral lower respiratory tract illness in children occurring during the first 2 years of life. Usually defined by its clinical manifestations. There is no standard management of bronchiolitis. We presented a report on management and outcomes of children with initial assessment of bronchiolitis admitted to Dr. Kariadi Hospital from January 2014 - December 2017. Case Presentation Summary : A total of 28 children were admitted to Dr. Kariadi Hospital from 2014 - 2017 due to bronchiolitis based on clinical symptoms and signs. Male to female ratio was 1 : 1.5. The mean age was 5 months (1-21 months). Modified RDAI score labeled respiratory distress as mild in 64.2% patients, moderate in 32.2% patients, and severe in 3.6% patients. Laboratory investigation revealed elevated WBC count in 10/28 patients (35.7%), had infection manifestations, and received antibiotics. Oxygen was given to 50% patients due to SpO2 <90%. Eleven patients (39.2%) were given nebulized ?-adrenergic agonist combined with corticosteroid injection or oral, 6/28 patients (21.4%) were given nebulized ?-adrenergic agonist, 4/28 patients (14.4%) were given corticosteroid injection or oral, and 7/28 patients (25%) got symptomatic treatment. Nebulization was given regularly for 2 days. The mean length of hospital stay was 5 days (3-11 days). Twelve patients were well-nourished, 16 patients were malnourished and nutritional intervention were given. Patients given nebulized ?-adrenergic agonist combined with corticosteroid had better clinical improvement, less length of stay (4.5 vs 6 days ; p=0.403). Two patients, 1 of which had severe RDAI score, were hospitalized for 11 days due to acyanotic congenital heart disease. Learning Points/Discussion : There is no standard treatment protocols and guidelines of bronchiolitis, thus various management were given to hospitalized children. All of them had good outcome.
