THE ROLE OF MATERNAL AND NEONATAL FACTORS IN SUCCESSFUL EARLY INITIATION OF BREASTFEEDING

Background : Child health efforts are continuously evaluated to maintain SDGs targets. Although Infant Mortality Rate (IMR) in Bali Province has reached the target but its still high compared to other countries. Early Initiation of Breastfeeding (EIBF) has an invaluable benefit and it’s associated with lower infant mortality and childhood morbidity. We aim to assess the role of mother and infant factors for EIBF implementation. Material : We reviewed medical records of mothers and delivered infants at Wangaya Hospital, Denpasar–Bali, from January–April 2018. Results : A sample of 384 pairs of mother and infant were included. About 55,5% of mothers initiated breastfeeding within the first hour of childbirth. Chi-square tests had resulted the association between mode of birth (p=0,00) (i.e. spontaneous vaginal birth, caesarean and assisted vaginal birth), intrapartum risk (PR 2,6 95%CI 1,9–3,5; p=0,00), APGAR score (PR 2,9 95%CI 2,49–3,3;p=0,00), parity (p=0,03), gestational age (p=0,00) and birth weight (p=0,00) to the implementation of EIBF. >From 187 vaginal births, 154 infants received EIBF. Logistic regression analysis showed that vaginal birth (PR 33,7 95%CI 4–277; p=0,001), APGAR score (p=0,001) and intrapartum risk factors (PR 2,3 95%CI 0,1–3,3; p=0,05) were significantly associated in successful EIBF. Conclusions : Good classification had dedicated to the implementation of EIBF at Wangaya Hospital. The intrapartum and immediate postnatal period are the crucial time for promoting the success rate of breastfeeding implementation. After identification of the risk factors, adequate breastfeeding support should be initiated immediately. Fetomaternal screening, antenatal care, education, counseling and peer support intervention is needed for all the mothers to increase their understanding of breastfeeding and child illness also to reduce IMR and maternal health risks.
