Assocation between vitamin D deficiency with Hypertension in Children with Systemic Lupus Erythematosus

Background : Vitamin D deficiency is highly prevalence in systemic lupus erythematosus (SLE) patients due to the disease itself, photosensitivity, renal insufficiency and use of medications. Previous studies suggested that low concentration of 25-hydroxyvitamin D3 were associated with a higher risk of hypertension. However, the effect of vitamin D deficiency on hypertension in SLE patients has not been studied yet. This study aimed to determine the association between vitamin D consentration and hypertension in pediatric SLE patients
Material : We conducted a cross sectional study involved 62 childrens who were diagnosed with SLE in Dr. Sardjito General Hospital Yogyakarta. Our subjects were children aged 5 to 18 years who received SLE therapy protocol from Januari 2014 - April 2018. Vitamin D deficiency was defined as the consentration level of 25-hydroxyvitamin D3 < 20 ng/ml. The association between 25-hydroxyvitamin D3 concentrations and hypertension was analyzed using univariate and multivariate logistic regression adjusting for several variables including disease activity, steroid therapy, obesity, dyslipidemia, and renal function.
Results : Most of our subjects were female (93.5%) with mean of age 14.6 ± 3.1 years. The prevalence of vitamin D deficiency was 66%. Hypertension observed in 51.2% of patients with vitamin D deficiency and 66.7 % in non deficiency patients. Logistic regression analysis found that no significant association between vitamin D deficiency and hypertension (OR 2.4, 95% CI 0.81 – 7.19, p = 0.115).
Conclusions : Lower serum 25-hydroxyvitamin D3 concentrations had no association with hypertension in SLE patients
