THE CORRELATION BETWEEN ONSET OF INTRAARTICULAR CORTICOSTEROID INJECTION WITH TIME OF REMISSION IN JUVENILE IDIOPATHIC ARTHRITIS PATIENTS

Background : Corticosteroid intra articular injection (IAI) has been used as treatment for Juvenile Idiopathic Arthritis (JIA), to relief symptoms of active synovitis, including pain, swelling and impaired join mobility. This study was to determine the correlation between onset of corticosteroid IAI with time of remission in JIA patients
Material : This research used cross sectional methods. JIA patients treated by corticosteroid IAI since January 1st 2012 until March 31st 2018 in Sardjito General Hospital, Yogyakarta that diagnosed from ILAR criteria were included while patients who had other autoimmune diseases such as psoriasis, ankylosing spondylitis, systemic lupus erythematosus were excluded. Remission was determined by inactive criteria of Wallace. Onset of corticosteroid IAI was classified less or more than 3 months. Data were collected from medical records and analyzed with log rank (mantel-cox) test
Results : There were 36 JIA patients who got corticosteroid IAI. The mean age was 10 years, with range 0-16 years old. Fourteen (39%) patients were male, while 22 (61%) patients were female. Nineteen (53%) patients were oligoarticular JIA, 14 (39%) patients were poliarticular JIA with negative rheumatoid factor, 1(3%) patient was poliarticular JIA with positive rheumatoid factor and 2 (5%) patients were systemic JIA. Thirty (83%) had onset of corticosteroid IAI less than 3 months, while 6 (17%) had onset more than 3 months. Twenty eight (78%) patients had remission while 8 (22%) patients hadn’t remission. Time of remission varied between 0 until 29 months. Onset of corticosteroid intra-articular injection had correlation with time of remission in juvenile idiopathic arthritis patients. (p<0.05)
Conclusions : There was the significantly correlation between onset of corticosteroid intra-articular injection with time of remission in juvenile idiopathic arthritis patients.
