SENSORINEURAL HEARING STATUS IN HIV-INFECTED CHILDREN

Background : HIV infection in children might increase the risk for hearing loss (HL), including sensorineural hearing loss (SNHL). Sensorineural status data was limited in HIV-infected children in Indonesia. The aim of this study was to identified the prevalence of SNHL and myelin dysfunction in HIV-infected children treated with antiretroviral (ARV).
Material : We conducted a cross sectional study in previously treated HIV-infected children aged 7 to 17 years from February-March 2018. The subjects were children attending Pediatric Allergy-Immunology clinic in Cipto Mangunkusumo Hospital (CMH), Jakarta. All of them were vertically infected with the HIV virus from mother-to-child transmission. Viral load of HIV and absolute CD4+ cell counts were registered. Patients underwent brainstem evoked responses auditory (BERA) at Pediatric Neurology clinic in CMH. Sensorineural hearing was classified as either normal and SNHL. Abnormal prolongations of interwave intervals considered as myelin dysfunctions.
Results : Of the 63 children enrolled, 32 (50.8%) were female. Subjects median age and ARV medication duration were 11.5 years (range 8.0-16.6) and 9 years (range 4-13), respectively. Viral load less than 5,000 copies/mL was observed in 48 children (76.2%). Subjects median CD4 percentage were 30 (range 11-45). Sensorineural hearing loss was found in 3 subjects (4.8%). Thus, myelin dysfunction was observed in 9 children (14.3%). Interpeak latency prolongation I-III and III-V wave were observed in 7 subjects (11.1%) and 2 subjects (3.2%), respectively.
Conclusions : Sensorineural hearing loss and myelin dysfunction could occurred in antiretroviral-experienced HIV-infected children eventhough in small percentage. Early sensorineural hearing screening need to be done for identifying sensorineural hearing loss in HIV-infected children. So, they would get hearing rehabilitation as soon as possible.
