Background : Systemic lupus erythematosus (SLE) results in a dysfunction of the immune system. Many autoantibodies detected in this disease are Anti-ds DNA antibodies, ANA, and the complement system (C3, C4). The research aimed at explaining the association of immunological markers in determine the activities of SLE disease. Material : We conducted a retrospective cohort study from medical records of children with SLE aged 0 – 18 years at RSUP Dr. Sardjito during January until March 2018. Patients with incomplete medical records and immune system disorders were excluded. The diagnosis of SLE was established by using criteria of SLICC 2012. The study focused on examining the role of immunological markers (anti ds-DNA, ANA and the level of C3, C4) on the activities of SLE, activities were observed by Mex - Systemic Lupus Erytematosus Activity Index (SLEDAI) under the category of active and inactive SLE. Data were analyzed using Chi-square and logistic regression tests. Results : A total of 55 patients (1 male and 54 female) were involved with median of age was 14 years old (range 0 until 18 years old). 38 patients with ANA positive (88.4%), 36 patients with dsDNA positive (87.8%), 41 patients with C3 positive (91.1%) and 32 patients with C4 positive (91.4%). Bivariate analysis showed that C3 (P= 0.001) and C4 (P= 0.039) statistically significant in active SLE, on multivariate analysis C3 was the only significant marker immunology for SLE activities (active SLE) with OR = 10.250; 95% CI : 2.050-51.256; P= 0.005 Conclusions : Active SLE patients based on Mex-SLEDAI have significantly positive C3 value
