CORRELATION BETWEEN SYMPTOMS SEVERITY AND ENDOSCOPICAL ESOPHAGITIS GRADE IN CHILDREN WITH GASTROESOPHAGEAL REFLUX DISEASE

Background : The symptoms of gastroesophageal reflux disease (GERD) in children vary widely, making it difficult to predict GERD on a clinical basis. The primary aim of this study was to assess the relation between the symptom severity in patients suspected with GERD and the endoscopical esophagitis grade, through the use of validated and reliable scoring.
Material : Cross-sectional study was carried out from 2015 to September 2017 with consecutive sampling technique. Children age between 2 and 17 years with symptoms of GERD who came to pediatric ambulatory care of Cipto Mangunkusumo Hospital, Jakarta were reffered to perform upper gastrointestinal endoscopy. The children are divided into 2 groups; age 2-8 years and age 9-17 years. The main clinical symptoms and their impact on daily life and school activities were assessed using a validated Pediatric Gastroesophageal Symptom and Quality of Life (PGSQ) questionnaire. Los Angeles (LA) classification were used to grade esophagitis seen during endoscopy; it divides reflux esophagitis into four grades.
Results : A total 60 children were included in the study. The male to female ratio was 1 : 1.4 and and the median age was 8.5 ± (2.0 -17.0) years old. The distribution of age and sex between esophagitis and non esophagitis groups did not differ signficantly (p = 0.42; p = 0.37 respectively). The percentage of patients in grade A, B, C, D esophagitis and non esophagitis were 50%; 28.3%; 13.3%; 3.3% and 5% respectively. No significant correlation was found between PGSQ symptomatic score and LA Classification of esophagitis (r = -0.222; p = 0.08).
Conclusions : The main finding of this study was lack of correlation between symptom severity and endoscopical esophagitis grade. Clinical symptoms can not reflect the severity of mucosal damage.
