Helicobacter pylori infection prevalence among symptomatic children in Yogyakarta, Indonesia

Background : Since current evidence indicates that Helicobacter pylori infection in children does not cause symptoms in the absence of gastric or duodenal ulceration, EPSGHAN/NAPSGHAN state against “test and treat” strategy and recommend upper gastrointestinal endoscopy as main modality of investigation in the clinical setting. Several limited studies shows various H. pylori infection prevalence among children in different region in Indonesia, however there was no publication of its prevalence among child with gastrointestinal symptoms and using endoscopic-based diagnosis. The aim of study was to report the prevalence of H. pylori among children with gastrointestinal symptoms in Yogyakarta, Indonesia.
Material : We conducted a cross sectional study on medical record of pediatric gastrohepatology clinic patients of Sardjito Hospital, Yogyakarta, Indonesia. Data were collected from 3-18 years old patients with gastrointestinal symptoms diagnosed as peptic ulcer diseases (gastritis or duodenitis) or childhood functional gastrointestinal disorder criteria with alarm signs (ROME IV criteria), who underwent upper gastrointestinal endoscopy and histologic examination for H. pylori within 2014-2017.
Results : A total of 125 patient record were included. The primary symptoms were recurrent abdominal pain (71%, 89/125), recurrent vomiting (21%, 26/125) and gastrointestinal bleeding (4%, 5/125). H. pylori were found on 22 (17,6%) patients. There were no difference in symptoms characteristics between infected and non infected children.
Conclusions : We reported relatively low prevalence of H. pylori infection among children with gastrointestinal symptoms in Yogyakarta, Indonesia
