IMMUNOPHENOTYPIC PROFILE OF CHILDREN ACUTE LYMPHOBLASTIC LEUKEMIA IN MOHAMMAD HOESIN HOSPITAL PALEMBANG

Background : Immunophenotyping in acute lymphoblastic leukemia (ALL) has been applied for examining the role of membrane antigen expression in predicting treatment response. This study aims to describe immunophenotypic profile of children ALL in Mohammad Hoesin General Hospital Palembang. Material : We retrospectively reviewed the immunophenotyping results of newly diagnosed children with ALL after initiation of induction phase chemotherapy between January 2015 and December 2016. Patients who had CD10, CD19, CD20, antigen in various combination are classified as B-lineage ALL, while patients with CD3, CD5, and CD7 antigen are classified as T-lineage ALL. Results : There were 40 newly diagnosed ALL patients (29 boys and 11 girls) who had immunophenotyping results, with age ranged from 1-15 years old. Thirty-one patients were classified as B-lineage and nine patients as T-lineage ALL. Thirty subjects had remission after induction phase of chemotherapy. Ten patients deceased during induction phase, all classified as B-lineage. Most of the patients who died also had positive marker of stem cell/hematopoietic precursor, 8/10 had HLA-DR and 7/10 had CD34 antigen. We also found an aberrant expression of myeloid marker consisting of CD33 and CD13 in 7/40 and 8/40 cases respectively. Only 9/33 patients with HLA-DR antigen and 9/32 patients with CD19 antigen had hyperleukocytosis. Conclusions : B-lineage ALL with an expression of stem cell/hematopoietic precursor marker appeared to have worse prognosis. A larger study is needed to better understand the role of membrane antigen expression in predicting treatment response of ALL.
