CLINICAL CHARACTERISTICS AND SURVIVAL ANALYSIS OF CHILDREN OVER 10 YEARS OF AGE WITH ACUTE LYMPHOBLASTIC LEUKEMIA AT DR SARDJITO HOSPITAL

Background : Acute lymphoblastic leukemia (ALL) is the most common malignancy in children and represents 75-80% of acute leukemia in this age group. Adolescents and young adults (AYA) with acute lymphoblastic leukemia have a substantial difference in disease biology and response to therapy with event-free survival of only 30-45%. The purpose of this study was to explore the clinical characteristics and survival analysis of children over 10 years of age with acute lymphoblastic leukemia (ALL) using Indonesian ALL 2013 Protocol.
Material : A total of 41 newly diagnosed ALL children over 10 years of age using Indonesian ALL 2013 Protocol were enrolled. Data were collected from medical record. Clinical characteristics and survival analysis were retrospectively analyzed. Event-free survival (EFS) and overall survival (OS) rates were presented by the Kaplan-Meier curve.
Results : Of the total 41 subjects diagnosed with ALL over 10 years of age, 19 patients were females , 22 patients were males, 28 patients were >10-15 years of age, and 13 patients were >15-18 years of age. At diagnosis, 4 patients were severely underweight, 15 patients had hemoglobin of <7 gr/dl, 21 patients showed high leukocyte counts (>=50x109/L), 23 patients had lymphadenopathies, 30 patients had splenomegaly, and 35 patients had hepatomegaly. After completing the induction phase, 17 patients achieved complete remission. Eight patients did not finish induction phase which consisted of 5 dropouts and 3 deaths. The 2 years event-free survival and overall survival for all 41 patients was 24.4% and 42.1% respectively.
Conclusions : Children with ALL over 10 years of age have a clinical characteristics of unfavorable prognosis. EFS and OS were low , therefore further evaluation and new protocol are needed.
