How to promote a smooth journey for extreme low birth weight premature babies

Background : With the advanced technology applied in nenonatal intensive care, survival rate for extreme low birth weight babies becomes increasing , and it is a long and difficult journey for these babies since their birth. To promote a smooth journey for them, it depends on the collaboration and cooperation between multidisciplines.
Material : A restropective study in a neonatal intensive care unit (NICU) of a regional hospital in Hong Kong has been conducted for the period from 2011-2016. It recruited newborns of gestation age below 28 weeks, and multidisciplinary approach is delivered to this group of babies starting before their births throughout the hospitalization to follow up after discharge home. Hospital data and patient’s records were reviewed for the study.
Results : Total 178 newborns of gestation age ranged 21-28 weeks with birth weight 510-920 grams were recruited. The survival rate was at 35-87%. Common morbidity for the babies was neuro-developmental disabilities that there was 60% in babies of 23 weeks whereas 15% in babies at 25 weeks. It was the lower gestation age at birth the higher chance of neuro-developmental problems for the babies. After implementation of intensive treatment programs including outreach resuscutation to labour room, developmental care and early discharge program, and follow up developmental training after discharge, it revealed that follow up at 6-month time, 5-12% of babies born before 28 weeks had severe developmental delay while there was 8-14% at 12 -month time.
Conclusions : Survival rate for extreme low birth weight premature babies is increasing and it is similar as the rate found in other overseas countries. Intensive treatment and care programs provided during hospitalization together with intensive developmental training given after discharge home may promote a smooth journey for this group of babies.
