RISK FACTORS FOR RELAPSE IN PEDIATRIC NEPHROTIC SYNDROME

Background : Nephrotic syndrome (NS) is the most common kidney disease in children characterized by edema, massive proteinuria, hypoalbuminemia, and hyperlipidemia. High relapse rate remains as a major problem in the management of this syndrome. This study aimed to identify the risk factor of relapse in pediatric nephrotic syndrome.
Material : This case control study was carried out in the Wahidin Sudirohusodo Teaching Hospital Makassar, Indonesia from January to August 2017 using complete medical records of patients diagnosed as having NS. The enrolled cases were divided into 2 groups : 1) relapsing NS group and 2) non-relapsing NS group. The following risk factors for relapse including age, sex, nutritional status, hypertension, serum creatinine levels, and infection at the time of established diagnosis of NS was analyzed using Chi-square (X2) test.
Results : A total of 142 children with NS who fulfilled the inclusion criteria aged 1.4 to 17.5 years mostly boys (66,2%) with a boy to girl ratio of 1,95:1 was evaluated. There were 80 cases (56,3%) of relapsing NS group and 62 cases (43,7%) of non-relapsing NS group. Statistical analysis showed that nutritional status was a significant risk factor for relapse with the value of p=0,023 (p <0,05).
Conclusions : Nutritional status was an independent risk factor of relapse in childhood nephrotic syndrome.
