NEUTROPHIL-LYMPHOCYTE RATIO AS A MARKER OF URINARY TRACT INFECTION IN CHILDREN CAUSED BY BACTERIA

Background : Urinary tract infection (UTI) is the most common bacterial infection that causes an inflammatory response in children. The gold standard for diagnosis of UTI is urine culture, but the result takes longer, so it has been studied several parameters to predict rapidly the cause of UTI which to be used as guideline in the treatment of UTI. The neutrophil-lymphocyte ratio (NLR) is easy to calculate and obtained based on routine blood tests at an affordable price so that it can be used at limited health resources. The aim of this study is to determine the NLR as a marker of UTI in children caused by bacteria.
Material : This was cross sectional study with a specificity on diagnostic tests of patients with suspected UTI in Adam Malik and USU General Hospital Medan from September 2017 to January 2018. NLR measurements were performed if urine culture was positive result. The NLR cut off point is determined based on the receiver-operating characteristic (ROC) curve.
Results : This study enrolled 35 from 50 children who met the inclusion criteria. ROC analysis of NLR as a marker of bacterial UTI in children obtained NLR cut off point of 4.55. Statistical analysis showed an insignificant NLR value (p = 0.073) to be used as a UTI marker in children caused by bacteria. The ROC curve showed an area under curve (AUC) of 0.665 (CI 95%: 0.515-0.815).
Conclusions : NLR can not be used as a UTI marker in children caused by bacteria eventhough this tests can be used at limited health resources.
