VALIDATION OF BRIGHTON SCORE IN CHILDREN WITH GUILLAIN BARRE SYNDROME AT Dr SOETOMO HOSPITAL SURABAYA

Background : Guillain Barre Syndrome (GBS) is an acute polyradiculoneuropathy with a variable case definitions. Several diagnostic criteria for GBS have been proposed, but none is accepted worldwide. The novel diagnostic criteria is Brighton Score. The aim of this study is to validate the sensitivity of Brighton Score for diagnosis of GBS. Material : A retrospective study of children with GBS was done in Dr. Soetomo Hospital. Surabaya from April 2016 to April 2018. Miller Fisher Syndrome was excluded. Medical records and the data related to age, sex, electromyelography (EMG) and lumbal pucture result, motor impairment and variant of GBS were obtained. Sensitivity of the Brighton Criteria for level 1, 2, 3, and 4 certainty were calculated. Results : Total 51 children with GBS was enrolled in this study. The mean age was 7.9 (SD 3.7) years, which 52.9% were male. EMG were performed in all cases, 32 (62.7%) were AIDP (Acute Inflammatory Demyelinating Polyneuropathy). Lumbar puncture was done only in 10 patient, in mean time 8.4 days (SD 2.1), all showed albuminocytological dissociation. Brighton criteria for level 1 was only fulfilled in 10 patient (19.6%), Level 2 in 41 patient (80.4%), Level 3 and 4 in none. Conclusions : Brighton score certainty level 2 were fulfilled in majority GBS children. Routine lumbar puncture in the future had to be done to increase sensitivity.
