PREDICTIVE VALUE OF TOPRS SCORE IN PATIENTS THROUGH TO PEDIATRIC EMERGENCY INSTALATION

Background : Scoring of severity of illness in pediatric emergency is rarely found. TOPRS scores is new scoring system using Systemic Inflammatory Response Syndrome (SIRS) physical sign and criterias of Advanced Pediatric Live Support (APLS). The aims of this study to evaluate TOPRS Scores of children admitted to pediatric emergency.
Material : This is a prospective cohort study on severity illness score in pediatric patient attending pediatric emergency unit at Pediatric Departement Wahidin Sudirohusodo Hospital from October to November 2016. Study population is all children admitted to pediatric emergency unit Wahidin Sudirohusodo Hospital. TOPRS scores were assessed since admission. Subjects were followed up until either one of two outcome groups the outcome occurred either recovered or died group. Predictive ability of this scorring system was analyzed using ROC curve.
Results : Among 218 subjects, 46 were died, and 172 were recovered. TOPRS scores were higher in non survivor group, with the optimal cut off value ?2 to distinguish survivor and no survivor group. p value 0.000, 100 sensitivity, 89,5 specificity, 100 PPV, 74 NPV. Multivariate analysis showed that TOPRS scoring system was independent variable and can be used as a parameter to evaluate with p value 0,000 (95% CI 0.00-). Conclusions : TOPRS scoring system has a predictive value in children admitted to pediatric emergency. Optimal cut off value to distinguish mortality is ?2.
