Advances in nutrition management in children with chronic kidney disease

Nutritional management of children with chronic kidney disease (CKD) is important and a challenge because children’s diet can affect how well the kidney’s work, children’s growth and the progression of kidney disease. Children with CKD required assessment of nutritional status and optimal nutritional management to prevent the development protein energy malnutrition (PEM), adequate growth and reduce progression of kidney disease. The goals nutritional care of children are maintenance of optimal status, avoidance of uremic toxicity, metabolic abnormalities, malnutrition and reduce of the risk of chronic morbidities and mortality. Five major nutritional components are considered such as energy, macronutrients, fluid and electrolytes, micronutrients and calcium/phosphorus/vitamin D. In children with CKD there is no one-size-fits-all diet that can be prescribed. Medical nutrition therapy needs to be individualized. Energy requirement for children with CKD should be considered the staging of CKD, hence continuous reassessment and readjustment are required as the child ages and CKD progress.
