Profile of Juvenile Idiopathic Arthritis Patients in Cipto Mangunkusomo Hospital, Jakarta

Background : Juvenile idiopathic arthritis (JIA) is a chronic arthritis in children, characterized by joint pain and inflammation. Diagnosis is based on clinical finding, laboratory results can help to exclude other diagnoses, categorize the disease and guide therapy. Appropriate treatment can induce remission and prevent joint destruction. Delay of treatment is associated with worse functional outcome, such as joint deformity, growth abnormalities, uveitis and limitations of functional capacity. This study aimed to determine the characteristics and outcome of JIA patients in Cipto Mangunkusumo Hospital.
Material : This was a descriptive study. Data was collected from the medical reports from January 2015 until December 2017.
Results : There were 32 JIA patients who were visited our clinic on the study period. Most of them were female (62.5%), with average age was 9.6 years old. Diagnosis of oligoarticular, polyarticular, and systemic JIA were 34.4%, 46.9% and 15.6%, and one patient (3.1%) with enthesitis JIA. There were 2 patients (6.2%) with positive rheumatoid factor and 22 patients (68.8%) with positive ANA. Treatment were mostly using methotrexate/MTX (78.1%), combination with methylprednisolone (18.7%), and diclofenac sodium or other NSAID. There were patients treated with leflunomide, combination of methotrexate-azathioprine, and methotrexate-sulfasalazine (one patient of each therapy). All of them underwent physiotherapy management and two of them needed steroid intraarticular injection. A few cases has developed complication such as deformity or contracture (12.5% patients) and one patient reported uveitis. Remission occured in 4 patients, and the rest mostly reduced in number of joint complain. There were 12.5 % patient who lost to follow up.
Conclusions : Most of our JIA patients had polyarticular arthritis with negative rheumatoid factor. Most of them were given MTX with good outcome. Complication such as deformity or contracture was developed in some patients.
