HISTOPATOLOGICAL PROFILE OF SYSTEMIC LUPUS ERYTHEMATOSUS AT WAHIDIN SUDIROHUSODO HOSPITAL FROM 2011 - 2018

Background : Systemic lupus erythematosus (SLE) is a relatively rare multisystemic autoimune disorder in childhood. However, limited data regarding SLE among pediatrics cohorts have been published. This retrospective study aimed to describe the renal histological characteristics of children with SLE in a 7-year period. Material : Fourty two children admitted to the wahidin sudirohusodo hospital between 2011 until 2018 were identified to manifest SLE on the basis of the international criteria established by the American College oh Rheumatology. Renal biopsy were underwent to 9 of these 42 patients. Renal lesions were classified according to the World Health Organization classification (WHO) : class II: pure mesangiale proliferative LN; class III: focal segmental proliferative glomerulonephritis LN; class IV: diffuse glomerulonephritis LN and class V: diffuse membranous glomerulonephritis LN. Results : The study population consisted in 42 children with SLE. There were 39 females and three males (The female to-male ratio: 13/1). The median age of the children was 12 years (range: 8-17 years). Pericarditis (33%), musculoskeletal (61%) and mucocutaneous (28%) involvement were the most common extra renal presentation. Seizure was found in 17 patients (40%). Concerning renal manifestations, hematuria was present in 27 patients (64.3%), proteinuria in 24 patients (57%). mean eGFR was 88 ml/min per 1.73 m2. The most frequent histopathological finding was class III and III-IV Lupus Nephritis 22,2%( 2 of 9), class IV 11,1% (1 of 9), class II 11,1% (1 of 9) and class I 11,1%(1 of 9) LN. There was no significant difference in LN presentation between histopathological groups. Conclusions : Renal involvement occurs in the majority of cSLE patients. The high incidence of proliferative lesion (clas III or IV) on renal biposy.
