Cardiac Metastases of Renal Cell Carcinoma ( A Very Rare Case

Background : Cardiac metastases from renal cell carcinoma are extremely rare with a limited number of cases reported in the worldwide literature until now. Nevertheless, this rare location of metastasis may significantly influence patient treatment and prognosis. For these individuals, treatment guidelines based on large scale studies are unavailable and only case analysis may provide clinicians with decision assistance. The objective of this study is to report a case of cardiac metastases from renal cell carcinoma.
Case Presentation Summary : We report the case of a 12-year-old girl who was diagnosed with renal cell carcinoma by an ultrasound-guided renal biopsy. Abdominal computed tomography demonstrated a right renal mass related to the probable neoplasm. The patient was started to have chemotherapy which was poorly tolerated. One month after chemotherapy, she experienced shortness of breath and ascites, with clinical sign of congestive heart failure. Echocardiography revealed a large right atrial mass (3.0mm x 3.98mm) from inferior vena cava and reduced ventricular function (ejection fraction 23,8%). However, after finished the chemotherapy cycle and treated with anti-congestive medication, reevaluation showed the ventricular function was improved (ejection fraction 43,6%), cardiac mass became smaller(2.3mm x 2.5mm), and no sign of congestive heart failure.
Learning Points/Discussion : Cardiac metastases present a unique disease course in renal cell carcinoma. Targeted systemic therapy with chemotherapy may be useful for such patients.
